How to make a Subject Access Request

You can apply to see information we hold about you. Requests are known as 'subject access requests'. Download and complete the subject access request and we will provide all the current data we hold.

Proof of Data Subject’s Identity

Although you can make verbal/written request, we still require proof of your identity before we can respond to your access request.

To help us establish your identity, you must provide identification that clearly shows your name, date of birth, and current address. We will accept a photocopy or a scanned image of the following as proof of identity: passport, driver’s licence or birth or adoption certificate. Please also attach a copy of a bank or credit card statement or utility bill showing your current address and dated within the last three months. If you have changed your name, please provide the relevant documents evidencing the change. You can also bring copies into our offices where our Customer Service Point will take copies (see http://www.westlancs.gov.uk/about-the-council/contact-us.aspx for relevant addresses).

We may request additional information from you to help confirm your identity and your right to access and we reserve the right to refuse to act on your request if we are unable to identify you.

We expect to respond to your request within one calendar month on receipt of the information you are seeking as well as providing proof of identity. However, we may sometimes need to extend our timescale to respond to a total of three months where your request is complex or forms part of a number of requests made by you.
